ABSTRACT

Abstract Objective Lower urinary tract symptoms (LUTS) affect approximately 10% of children worldwide and are related to psychosocial manifestations and compromised quality of life, both for children and their families. The assessment of emotional conditions of LUTS in children is recommended by International Children's Continence Society; however, there is no specific instrument in the Brazilian Portuguese language. Therefore, the aim of this study was to translate, culturally adapt and assess the internal consistency of the Brazilian Portuguese version of the Pediatric Incontinence Questionnaire (PINQ). Material and methods This cross-sectional study was performed at two referral centers for childhood voiding dysfunction. The 20-item PINQ was translated into Brazilian Portuguese and culturally adapted according to Beaton, 2000. His-standard methodology consists of 6 phases: translation, synthesis, back-translation, expert committee, and pre-test. The internal consistency was assessed using Cronbach's alpha. Results The PINQ-br version was developed, validated by a committee of experts, and pre-tested on 44 children diagnosed with lower urinary tract symptoms, 23 boys and 21 girls (mean age: 9.7 and 9.6 years old respectively), as well as on their parents. The internal consistency was considered satisfactory, reaching Cronbach's alpha coefficient of 0.74 when applied to children and 0.82 when applied to parents. Conclusions The PINQ was translated and culturally adapted to Brazilian Portuguese to assess the impact of LUTS on the health-related quality of life in Brazilian children and adolescents.

Introdução: A gestação e o diabetes mellitus promovem diversas alterações musculoesqueléticas, predispondo disfunções miccionais e sexuais. Objetivo: Avaliar a frequência de incontinência urinária e disfunção sexual em gestantes diabéticas tipo 1. Métodos: Estudo analítico, observacional do tipo transversal. Foram avaliadas 13 gestantes com diabetes do tipo 1 no período de abril 2017 a fevereiro 2018, por meio dos Questionários International Consultation on Incontinence Questionnaire - Short Form (ICIQ-SF) e o Female Sexual Function Index (FSFI). Foi utilizado o programa estatístico SPSS versão 20.1 para análise de dados, avaliando a média e desvio padrão (DP), tendência central e dispersão, frequência absoluta (n) e relativa (%). Para correlação entre o ICIQ e o FSFI o teste de correlação linear de Pearson. Resultados: Frequência de incontinência urinária foi 38,5%, o que demonstrou impacto leve na qualidade de vida das gestantes. O escore geral do FSFI foi de 20,49, identificando a presença de disfunção sexual (FSFI ≤ 26). No grupo de gestantes, 92,3% (n = 12) apresentaram disfunção sexual, os domínios satisfação e excitação com maiores prevalências. Correlacionando o ICIQ-SF com FSFI, houve correlação moderada, mas não significativa (r = 0,534; p = 0,60). As amostras apresentaram distribuição normal de acordo com o teste de Levene. Conclusão: A frequência de incontinência urinária em gestantes diabéticas do tipo 1 foi baixa, mas houve presença de disfunção sexual, os domínios satisfação e excitação foram os mais frequentes.

ABSTRACT Purpose: to verify the effects of biofeedback (BF) and manual therapy (MT) associated with transcutaneous electrical nerve stimulation (TENS) or postural exercises (PE) in the treatment of bladder pain syndrome (BPS) in women regarding pain and urinary symptoms. Materials and Methods: a parallel-randomized controlled trial was conducted in BPS patients diagnosed according to NIH clinical criteria. Two specialized physiotherapists applied demographic and validated questionnaires of perineal and suprapubic pain (VAS), urinary symptoms and problems (ICSI and ICPI) and sexual function (FSFI) and a physical assessment was made to identify myofascial trigger points. Thirty-one women, mean age 51.8 ± 10.9 were randomized in three groups of treatment consisting of ten weekly sessions of BF and MT (Conventional group); BF, MT, and TENS (TENS group); and BF, MT, and PE (Postural group). Results: Postural group improved perineal and suprapubic pain after treatment (p<0.001 and p=0.001, respectively), and the suprapubic pain improvement remained persistent at 3 months of follow up (p=0.001). Postural group improved urinary symptoms and problems after treatment (p<0.001 and p=0.005, respectively) and during follow up (p<0.001 and p=0.001). Conclusions: Biofeedback and manual therapy associated with postural exercises showed a significant improvement in perineal and suprapubic pain and urinary symptoms after treatment and during follow-up. Both results suggest a possible role for the use of this physiotherapy technique to treat BPS patients. Longer follow-up and a larger number of patients are necessary to confirm these conclusions.

Abstract Importance: Despite ambulation capacity being associated with a decreased level of physical activity and survival may be influenced by the functional capacity, studies have not addressed the association between ambulation capacity and death in patients hospitalized by COVID-19. Objective: To verify the functional, clinical, and sociodemographic risk factors associated with in-hospital death in individuals with severe COVID-19. Methods: It is a cohort retrospective study performed at a large tertiary hospital. Patients 18 years of age or more, of both sexes, hospitalized due to severe COVID-19 were included. Cases with dubious medical records and/or missing essential data were excluded. Patients were classified according to their ambulation capacity before the COVID-19 infection. Information regarding sociodemographic characteristics, in-hospital death, total hospital stays, Intensive Care Unit (ICU) stays, and the necessity of Mechanical Ventilation (MV) were collected from medical records and registered in a RedCap database. Multiple logistic regression analysis was used to identify possible factors associated with the in-hospital death rate. Results: Data from 1110 participants were included in the statistical analysis. The median age of the patients was 57 (46‒66) years, 58.42% (n = 590) were male, and 61.73% (n = 602) were brown or black. The case fatality rate during hospitalization was 36.0% (n = 363). In-hospital death was associated with ambulation capacity; dependent ambulators (OR = 2.3; CI 95% = 1.2-4.4) and non-functional ambulation (OR = 1.9; CI 95% = 1.1-3.3), age [older adults (OR = 3.0; CI 95% = 1.9‒4.), ICU stays (OR = 1.4; CI 95% = 1.2‒1.4), immunosuppression (OR = 5.5 CI 95% = 2.3‒13.5) and mechanical ventilation (OR = 27.5; CI 95% = 12.0-62.9). Conclusion and relevance: Decreased ambulation capacity, age, length of ICU stay, immunosuppression, and mechanical ventilation was associated with a high risk of in-hospital death due to COVID-19.

INTRODUCTION: Gastroschisis is a congenital malformation that has risk factors for delayed neuropsychomotor development. That is why it is important to recognize early developmental changes in these newborns during hospitalization in the Neonatal Intensive Care UnitOBJECTIVE: To evaluate the motor performance of newborns with gastroschisis after surgical correctionMETHODS: An observational and retrospective study was carried out with newborns with gastroschisis, assessed by the Test of Infant Motor Performance at the Neonatal Intensive Care Center 2 of the Instituto da Criança e do Adolescente. The test allows the classification of the risk for developmental delay in 4 categories: within the average for age, low average, below average, and well below average. Maternal, neonatal, surgical, and motor assessment data were collected through electronic medical recordsRESULTS: Motor assessment was performed on 17 newborns, where 88.23% were classified as "below average" for age. The mean maternal age was 20 years, and the average gestational age and birth weight were 36.38 weeks and 2343.9 grams, respectively, with the majority being female. Simple gastroschisis accounted for 64.71%, and primary closure was possible in 82.35%. The average hospital stay of 53.24 days, and sepsis was the most frequent complication (64.71%CONCLUSION: Newborns with gastroschisis remain hospitalized for a long time and are susceptible to complications. It was possible to identify the delay in the development of these newborns early during hospitalization, which allows intervention by physiotherapy before the delay worsens


INTRODUÇÃO: A gastrosquise é uma malformação congênita que possui fatores de risco para o atraso no desenvolvimento neuropsicomotor. Por isso é importante o reconhecimento precoce das alterações do desenvolvimento nesses recém-nascidos durante a internação na Unidade de Terapia Intensiva NeonatalOBJETIVO: Avaliar o desempenho motor de recém-nascidos com gastrosquise após correção cirúrgicaMÉTODO: Foi realizado um estudo observacional e retrospectivo com recém-nascidos com gastrosquise, avaliados pelo Test of Infant Motor Performance no Centro de Tratamento Intensivo Neonatal 2 do Instituto da Criança e do Adolescente. O teste permite a classificação do risco para o atraso do desenvolvimento em 4 categorias: dentro da média para idade, média baixa, abaixo da média e muito abaixo da média. Foram coletados dados maternos, neonatais, cirúrgicos e da avaliação motora, através de prontuários eletrônicosRESULTADOS: A avaliação motora foi realizada em 17 recém-nascidos, onde 88,23% foram classificados como "abaixo da média" para idade. A idade média materna foi de 20 anos e a média de idade gestacional e peso ao nascimento de 36,38 semanas e 2343,9 gramas, respectivamente, sendo a maioria do sexo feminino. As gastrosquises simples representaram 64,71% e o fechamento primário foi possível em 82,35%. A média de permanência hospitalar de 53,24 dias e a sepse foi a complicação mais frequente (64,71%CONCLUSÃO: Os recém-nascidos com gastrosquise permanecem internados por tempo prolongado e estão suscetíveis a complicações. Foi possível a identificação precoce do atraso no desenvolvimento desses recém-nascidos durante a internação

ABSTRACT

INTRODUÇÃO: As ataxias cerebelares são um extenso grupo de doenças que causam diversos distúrbios na marcha e no equilíbrio, e que comprometem seriamente a qualidade de vida, sem opções de tratamento eficazes. A cinesioterapia é a base de programas multifacetados que incorporam mais de um enfoque, como o treinamento de coordenação e equilíbrio. Recentemente, a estimulação transcraniana por corrente contínua (tDCS) sobre o cerebelo surgiu como uma intervenção para melhorar os distúrbios do equilíbrio. OBJETIVO: Descrever a aplicação simultânea de tDCS anódica cerebelar e cinesioterapia, em sessões múltiplas diárias para reabilitação da ataxia cerebelar. MATERIAIS E MÉTODOS: Este relato de caso incluiu um paciente do sexo masculino, de 34 anos, com história de ataxia espinocerebelar há 10 anos. Seus principais objetivos eram melhorar a marcha e o equilíbrio. Ele apresentava ataxia axial e apendicular, dificuldades na marcha e no equilíbrio. O protocolo de estimulação do cerebelo consistiu na aplicação de tDCS por 20 minutos, 2mA, diariamente, durante duas semanas, com ânodo posicionado sobre o ínion e cátodo sobre o músculo deltóide direito. A cinesioterapia simultânea incluiu exercícios funcionais progressivos com objetivo principal de treinamento de equilíbrio. RESULTADOS: A melhora clínica foi particularmente evidenciada por uma redução de 4 pontos na Escala para Avaliação e Graduação da Ataxia após 10 sessões, enquanto a literatura recomenda a eficácia de uma nova terapia que retardaria a progressão da ataxia em 1 ponto por ano. CONCLUSÃO: Nossos resultados sugerem que a associação entre tDCS e cinesioterapia foi eficaz neste paciente; as sessões de tDCS foram seguras e bem toleradas e podem ter desempenhado um papel na melhora nos testes funcionais. Novos estudos controlados envolvendo um número maior de pacientes são necessários para analisar os benefícios destas técnicas combinadas para maximizar a reabilitação motora nesta população.


INTRODUCTION: Cerebellar ataxias are an extensive group of diseases, which cause many disorders in gait and balance that seriously impair quality of life, and without effective treatment options. Kinesiotherapy is the basis of multifaceted programs that incorporate more than one focus, such as coordination and balance training. Recently, transcranial direct current stimulation (tDCS) over the cerebellum has emerged as an intervention to improve balance disorders. OBJECTIVE: To describe a daily multiple session's simultaneous application of anodal cerebellar tDCS to kinesiotherapy for rehabilitation in cerebellar ataxia. MATERIALS AND METHODS: This case report included a 34-year-old male patient with a 10-year history of spinocerebellar ataxia. His main goals were to improve his walking ability and balance. He presented with axial and appendicular ataxia, impaired gait, and balance. The protocol used to stimulate the cerebellum consisted of twenty-minute tDCS, 2mA, daily applied, over two weeks, with anode positioned over the inion and cathode over the right deltoid muscle. Simultaneous kinesiotherapy included progressive functional exercises with the main objective of balance training. RESULTS: Clinical improvement was particularly evidenced by a 4-point reduction in the Scale for the Assessment and Rating of Ataxia after ten sessions, while literature recommends efficacy of a new therapy that would retard ataxia progression by 1 point per year. CONCLUSION: Our results suggest that the association between tDCS and kinesiotherapy was effective in this patient; tDCS sessions were safe and well-tolerated and may have played a role in improving functional tests. Further controlled studies involving a larger number of patients are needed to analyze the benefits of these combined techniques to maximize motor rehabilitation in this population.

OBJECTIVE: Coronavirus disease 2019 (COVID-19) is associated with high mortality among hospitalized patients and incurs high costs. Severe acute respiratory syndrome coronavirus 2 infection can trigger both inflammatory and thrombotic processes, and these complications can lead to a poorer prognosis. This study aimed to evaluate the association and temporal trends of D-dimer and C-reactive protein (CRP) levels with the incidence of venous thromboembolism (VTE), hospital mortality, and costs among inpatients with COVID-19. METHODS: Data were extracted from electronic patient records and laboratory databases. Crude and adjusted associations for age, sex, number of comorbidities, Sequential Organ Failure Assessment score at admission, and D-dimer or CRP logistic regression models were used to evaluate associations. RESULTS: Between March and June 2020, COVID-19 was documented in 3,254 inpatients. The D-dimer level ≥4,000 ng/mL fibrinogen equivalent unit (FEU) mortality odds ratio (OR) was 4.48 (adjusted OR: 1.97). The CRP level ≥220 mg/dL OR for death was 7.73 (adjusted OR: 3.93). The D-dimer level ≥4,000 ng/mL FEU VTE OR was 3.96 (adjusted OR: 3.26). The CRP level ≥220 mg/dL OR for VTE was 2.71 (adjusted OR: 1.92). All these analyses were statistically significant (p<0.001). Stratified hospital costs demonstrated a dose-response pattern. Adjusted D-dimer and CRP levels were associated with higher mortality and doubled hospital costs. In the first week, elevated D-dimer levels predicted VTE occurrence and systemic inflammatory harm, while CRP was a hospital mortality predictor. CONCLUSION: D-dimer and CRP levels were associated with higher hospital mortality and a higher incidence of VTE. D-dimer was more strongly associated with VTE, although its discriminative ability was poor, while CRP was a stronger predictor of hospital mortality. Their use outside the usual indications should not be modified and should be discouraged.

ABSTRACT OBJECTIVE To estimate the trends of fall-related hospitalization, mortality, and lethality among older adults in Brazil and regions. METHODS This is a descriptive study based on data from the Hospital Information System of the Brazilian Unified Health System. We included records of every older adult, aged 60 years or older, hospitalized for accidental fall from January, 1998 to November, 2015 in all Brazilian regions. We selected the codes E885, E886, E880, E884, E884 from the International Classification of Diseases, 9th revision, and W01, W03, W10, W17, W18 from the 10th revision, and calculated fall-related hospitalization and mortality rates per 100,000 inhabitants, as well as lethality. To estimate trends, we applied the Prais-Winsten regression for time series analysis. RESULTS During the period, 1,192,829 fall-related hospitalizations occurred, among which 54,673 had a fatal outcome; lethality was 4.5%. Hospitalization rates showed upward trends, with seasonality, in Brazil (11%), and in the Northeast (44%), Midwest (13%), and South regions (14%). The North showed a decreasing hospitalization rate (48%), and the Southeast a stationary one (3%). CONCLUSIONS In Brazil, fall-related hospitalizations, mortality, and lethality among older adults showed an upward trend from 1998 to 2015, with seasonal peaks in the second and third quarters. Considering we are in plain demographic transition, to improve hospital healthcare and encourage falls prevention programs among older adults is essential.


RESUMO OBJETIVO Estimar as tendências de internação, mortalidade e letalidade por quedas em idosos no Brasil e regiões. MÉTODOS Estudo descritivo realizado a partir de dados do Sistema de Informações Hospitalares do Sistema Único de Saúde. Foram incluídos os registros de todos os idosos, a partir de 60 anos, internados por quedas acidentais entre janeiro de 1998 e novembro de 2015 em todas regiões do Brasil. Foram selecionados os códigos E885, E886, E880, E884, E888 e W01, W03, W10, W17, W18 da Classificação Internacional de Doenças (CID), 9ª e 10ª revisão respectivamente, e calculadas as taxas de internação e mortalidade por quedas por 100.000 habitantes, além da letalidade. Para cálculo de tendências, usou-se o procedimento de Prais-Winsten de autorregressão para análise de séries temporais. RESULTADOS Foram realizadas 1.192.829 internações por quedas no período, ocorreram 54.673 desfechos fatais, e a letalidade foi de 4,5%. As taxas de internação apresentaram tendências crescentes, com sazonalidade, no Brasil e nas regiões Nordeste, Centro-Oeste e Sul (taxas de crescimento de 11%, 44%, 13% e 14%, respectivamente). No Norte, a taxa de internação foi decrescente, e no Sudeste foi estacionária (taxas de crescimento: 48% e 3%). CONCLUSÕES Houve tendência crescente de internações, mortalidade e letalidade por quedas em idosos entre 1998 e 2015 no Brasil, com picos sazonais no segundo e terceiro trimestres. É necessário aprimorar tanto a assistência hospitalar quanto incentivar programas de prevenção de quedas em idosos, visto que estamos em plena transição demográfica.

ABSTRACT

OBJECTIVES: We designed a cohort study to describe characteristics and outcomes of patients with coronavirus disease (COVID-19) admitted to the intensive care unit (ICU) in the largest public hospital in Sao Paulo, Brazil, as Latin America becomes the epicenter of the pandemic. METHODS: This is the protocol for a study being conducted at an academic hospital in Brazil with 300 adult ICU beds dedicated to COVID-19 patients. We will include adult patients admitted to the ICU with suspected or confirmed COVID-19 during the study period. The main outcome is ICU survival at 28 days. Data will be collected prospectively and retrospectively by trained investigators from the hospital's electronic medical records, using an electronic data capture tool. We will collect data on demographics, comorbidities, severity of disease, and laboratorial test results at admission. Information on the need for advanced life support and ventilator parameters will be collected during ICU stay. Patients will be followed up for 28 days in the ICU and 60 days in the hospital. We will plot Kaplan-Meier curves to estimate ICU and hospital survival and perform survival analysis using the Cox proportional hazards model to identify the main risk factors for mortality. ClinicalTrials.gov: NCT04378582. RESULTS: We expect to include a large sample of patients with COVID-19 admitted to the ICU and to be able to provide data on admission characteristics, use of advanced life support, ICU survival at 28 days, and hospital survival at 60 days. CONCLUSIONS: This study will provide epidemiological data about critically ill patients with COVID-19 in Brazil, which could inform health policy and resource allocation in low- and middle-income countries.

ABSTRACT

Introdução: Este estudo apresenta uma revisão da literatura a respeito da dor pélvica gestacional, uma dor sentida entre as cristas ilíacas posteriores, as dobras glúteas e as articulações sacroilíacas, podendo irradiar para a região posterior da coxa e ocorrer junto com a dor na sínfise púbica. Objetivos: Fazer uma revisão da literatura, levantando estudos sobre quais intervenções fisioterapêuticas foram mais eficazes no tratamento de mulheres portadoras de dor pélvica gestacional sem associação à dor lombar, e propor um manual de orientação para essas pacientes. Métodos: Revisão da literatura, consultando-se artigos publicados entre os anos 2008 e 2019, nas bases de dados Pedro, Pubmed e Scielo. Resultados: Os resultados desta pesquisa apontam que os recursos fisioterapêuticos encontrados nos estudos amenizaram a dor pélvica gestacional e contribuíram para funcionalidade das gestantes; nenhum, porém, foi capaz de zerar as queixas das pacientes. Conclusão: Existem recursos fisioterapêuticos para a dor pélvica gestacional que reduzem a dor e melhoram a funcionalidade das gestantes; porém, pelo baixo número de estudos sobre essa dor considerada isoladamente da dor lombar, sugerem-se mais pesquisas sobre abordagens fisioterapêuticas que se mostrem mais efetivas para mulheres nessa condição. Introduction: This study is a literature review of gestational pelvic pain, a pain felt between the posterior iliac crests, gluteal folds and sacroiliac joints, which may radiate to the posterior thigh and may occur along with pain in the pubic symphysis. Objectives: To review the literature and to study which physiotherapeutic interventions were most effective in treating women with gestational pelvic pain without association with low back pain, and to propose a guidance manual for these patients. Methods: Literature review, consulting articles published between 2008 and 2019, in the Pedro, Pubmed and Scielo databases. Results: The results indicate that the physiotherapeutic resources softened gestational pelvic pain and contributed to the functionality of pregnant women; none, however, was able to clear the patients' complaints. Conclusion: There are physical therapy resources for gestational pelvic pain, which reduce pain and improve the functionality of pregnant women; however, due to the low number of studies on this pain considered separately from low back pain, further research on physiotherapeutic approaches that are more effective for women in this condition is suggested. ABSTRACT

Trata-se de um relato de caso abordando a experiência de um projeto de gestão desenvolvido na Divisão de Fisioterapia do Instituto Central do Hospital das Clí­nicas da Faculdade de Medicina da Universidade de São Paulo. A gestão de serviços de saúde é complexa e desafiadora, devido a fatores como as constantes transformações tecnológicas e terapêuticas, aumento da população idosa e de doenças crônicas, escassez de recursos e carência de gestão eficiente nas organizações. A definição de objetivos, estratégias e instrumentos de monitoramento para direcionar a organização e promover qualidade são determinantes nas ações gerenciais e no desempenho estratégico da empresa. Neste contexto, o projeto objetivou construir um mapa estratégico considerando o Balanced ScoreCard como diretriz gerencial e instrumento de análise do desempenho da Divisão de Fisioterapia. Reuniões estruturadas com membros da liderança de fisioterapia foram realizadas para definir a visão do futuro e objetivos estratégicos a médio e longo prazo, idealizados pela Divisão de Fisioterapia, tendo como base as perspectivas do Balanced ScoreCard, aprendizado e crescimento, processos internos, clientes, sustentabilidade financeira e social. O mapa estratégico foi construí­do em uma representação gráfica permitindo fácil compreensão da visão do futuro e objetivos estratégicos que irão direcionar o comportamento e o desempenho da Divisão de Fisioterapia, nos próximos anos. Além de direcionar as ações de melhorias da Divisão, o mapa estratégico elaborado também apresenta caracterí­sticas marcantes de alinhamento com o mapa estratégico institucional. (AU)


This is a case report addressing a management project developed in the Physiotherapy Division of the Central Institute of Hospital das Clínicas, Faculty of Medicine, University of São Paulo. Managing health services is complex and challenging due to factors such as constant technological and therapeutic changes, increase in the elderly population and chronic diseases, scarcity of resources, and lack of efficient organizational management. The definition of objectives, strategies, and monitoring tools to direct the organization and promote quality help to determine managerial actions and the strategic performance of the service. In this context, this project aimed to build a strategic map using the Balanced Scorecard as a managerial guideline and instrument of performance analysis for the Physiotherapy Division. Structured meetings with members of the physiotherapy leadership team were held to define the vision for the future and strategic medium - and long-term objectives. The strategic map was built as a graphical representation, allowing for easy understanding of the vision for the future and strategic objectives that will guide the behavior and performance of the Physiotherapy Division in the coming years. In addition to directing the improvement actions of the division, the strategic map also presents striking instances of alignment with the institutional strategic map.(AU)

INTRODUÇÃO: Todos os anos, milhões de dólares são gastos para equipar e manter os centros de esterilização hospitalar no mundo. Materiais de terapia respiratória são essenciais para o suporte de vida e têm um grande impacto nas complicações clínicas. Fornecer materiais de terapia respiratória com higiene apropriada em um hospital é um desafio para os gestores devido ao impacto clínico e de custos. OBJETIVO: Comparar os custos de diferentes métodos de abastecimento de materiais de fisioterapia respiratória em um hospital geral, de porte extra, terciário e público. MÉTODOS: Consistiu em um estudo observacional, baseado em revisão documental e análise crítica de indicadores de gestão hospitalar, e analisou materiais utilizados em fisioterapia respiratória, classificados como semicríticos, incluindo ressuscitador manual, nebulizador, kit para pressão positiva contínua nas vias aéreas, circuito ventilatório para ventilação mecânica não invasiva e invasiva. RESULTADOS: Comparado ao processo de aquisição de materiais descartáveis, o processo de abastecimento no Centro de Material e Esterilização (CME) institucional gera uma economia anual com valores calculados em US$ 289.679,26; em comparação com o CME terceirizado, o processo de suprimento no CME institucional economiza um montante de US$ 257.041,11 por ano. CONCLUSÃO: Este foi o primeiro estudo brasileiro a realizar uma análise de custos nesse contexto. O processo de abastecimento com a higienização de materiais de fisioterapia respiratória na CME-INST mostrou-se mais vantajoso com menores custos quando comparado ao processo de abastecimento com higienização na CME-EXT ou custo de aquisição de MD. A economia anual estimada considerando o consumo médio mensal neste estudo ultrapassou US$ 250.000,00. A análise de custos detalhada fornece informações cruciais e pode permitir uma melhor gestão financeira. [AU]


INTRODUCTION: Every year millions of dollars are expended to equip and maintain the hospital sterilization centers in the world. Respiratory therapy materials are essential to life support, and have a major impact on clinical complications. To provide respiratory therapy materials with appropriate hygiene in a hospital is a challenge for manager due to clinical and costs impact. OBJECTIVE: To compare costs of different methods of providing respiratory therapy materials in a general, extra, tertiary and public hospital. METHODS: It consisted of an observational study, based on document review and critical analysis of hospital management indicators, and analyzed materials used in respiratory therapy, classified as semi-critical products, including manual resuscitator, nebulizer, kit for Continuous Positive Airway Pressure (CPAP), ventilatory circuit for non-invasive and invasive mechanical ventilation. RESULTS: Compared to the supply process of disposable materials acquisition, the supply process in the Institutional Central Sterilization Supply Department generates an annual savings with values calculated at US$ 289,679.26; while compared to the Outsourced Central Sterilization Supply Department the supply process in the Institutional Central Sterilization Supply Department saves an amount of US$ 257,041.11 annually. CONCLUSION: This was the first Brazilian study to conduct a cost analysis in this context. The supply process with the cleaning of respiratory physiotherapy materials in the Institutional Central Sterilization Supply Department was more advantageous with lower costs when compared to the Outsourced Central Sterilization Supply Department or cost of acquisition of disposable materials. The annual savings estimated considering the average monthly consumption in this study exceeded US $ 250,000.00. Detailed cost analysis provides crucial information and can enable better financial management. [AU]

ABSTRACT Objective: To determine whether 24-h availability of physiotherapy services decreases ICU costs in comparison with the standard 12 h/day availability among patients admitted to the ICU for the first time. Methods: This was an observational prevalence study involving 815 patients ≥ 18 years of age who had been on invasive mechanical ventilation (IMV) for ≥ 24 h and were discharged from an ICU to a ward at a tertiary teaching hospital in Brazil. The patients were divided into two groups according to h/day availability of physiotherapy services in the ICU: 24 h (PT-24; n = 332); and 12 h (PT-12; n = 483). The data collected included the reasons for hospital and ICU admissions; Acute Physiology and Chronic Health Evaluation II (APACHE II) score; IMV duration, ICU length of stay (ICU-LOS); and Omega score. Results: The severity of illness was similar in both groups. Round-the-clock availability of physiotherapy services was associated with shorter IMV durations and ICU-LOS, as well as with lower total, medical, and staff costs, in comparison with the standard 12 h/day availability. Conclusions: In the population studied, total costs and staff costs were lower in the PT-24 group than in the PT-12 group. The h/day availability of physiotherapy services was found to be a significant predictor of ICU costs.


RESUMO Objetivo: Determinar se a disponibilidade de serviços de fisioterapia 24 h/dia reduz os custos de UTI comparada à disponibilidade padrão de 12 h/dia entre pacientes admitidos pela primeira vez na UTI. Métodos: Estudo de prevalência observacional, envolvendo 815 pacientes ≥ 18 anos de idade que estavam em ventilação mecânica invasiva (VMI) por ≥ 24 h e que tiveram alta de uma UTI para uma enfermaria em um hospital universitário terciário no Brasil. Os pacientes foram divididos em dois grupos de acordo com a disponibilidade de serviços de fisioterapia na UTI em h/dia: 24 h (PT-24; n = 332); e 12 h (PT-12; n = 483). Os dados coletados incluíram os motivos das internações hospitalares e das admissões na UTI; a pontuação Acute Physiology and Chronic Health Evaluation II (APACHE II); a duração da VMI; o tempo de permanência na UTI (TP-UTI); e o escore Ômega. Resultados: A gravidade da doença foi similar em ambos os grupos. A disponibilidade ininterrupta de serviços de fisioterapia foi associada a tempos menores de VMI e TP-UTI, bem como a menores custos (totais, médicos e com pessoal), comparada à disponibilidade padrão de 12 h/dia. Conclusões: Na população estudada, os custos totais e os custos com pessoal foram menores no grupo PT-24 do que no grupo PT-12. A disponibilidade em h/dia dos serviços de fisioterapia mostrou ser um preditor significativo dos custos de UTI.

Objetivo: Conhecendo-se o alto grau de complexidade que o paciente politraumatizado representa à equipe multiprofissional na elaboração e execução do seu plano assistencial na unidade de terapia intensiva (UTI), aliado à carência de evidencias sobre o tema, o presente estudo sugere um modelo de assistência fisioterapêutica precoce aos pacientes críticos politraumatizados com base na experiência clínica dos últimos anos. Método: O modelo foi elaborado a partir das práticas verificadas nos registros de 6388 sessões de fisioterapia realizadas em 198 pacientes internados entre dezembro de 2009 e setembro de 2011 em UTI especializada em politrauma. As atividades/cuidados foram inseridas no modelo após aprovadas em discussão com a equipe multiprofissional. Todos os pacientes atendidos tinham idade igual ou maior que 18 anos e eram vítimas de trauma grave de acordo com o Injury Severity Score (ISS). Resultados: O modelo proposto foi estruturado de forma que as atividades/cuidados da assistência fisioterapêutica fossem organizadas de acordo com a região corpórea lesada do paciente (traumatismo cranioencefálico, fraturas de face, fraturas de coluna, trauma torácico, trauma abdominal, fratura de pelve e fraturas de extremidades). A rotina da unidade apregoava discussões diárias com a equipe médica para se conhecer as particularidades de cada caso clínico, estabelecer meta terapêutica e traçar o programa de reabilitação. Conclusão: O modelo proposto se tornou rotina e consolidou a atuação fisioterapêutica na respectiva unidade assistencial. A equipe de fisioterapia passou a atuar 24 horas por dia. O modelo possibilitou padronização da assistência fisioterapêutica e maior segurança para o paciente politraumatizado grave


Objective: In view of the high degree of complexity that the polytrauma patient represents to the multiprofessional team in the elaboration and execution of their care plan in the intensive care unit (ICU), combined with the lack of evidence on the subject, this study suggests a model of early physiotherapeutic assistance to critical polytrauma patients based on the clinical experience of recent years. Method: The model was elaborated based on the practices verified in the records of 6,388 physiotherapy sessions performed in 198 patients hospitalized between December 2009 and September 2011 in polytraumas-specialized ICU. The activities/care were inserted in the model after approved in discussion with the multiprofessional team. All patients enrolled were aged 18 years or older and were victims of severe trauma according to the Injury Severity Score (ISS). Results: The proposed model was structured in such a way that the physical therapy activities/care were organized according to the injured body region (traumatic brain injury, face fractures, spine fractures, thoracic trauma, abdominal trauma, pelvic fracture and extremities fractures). The routine of the ICU encouraged daily discussions with the medical team to know the particularities of each clinical case, to establish therapeutic goal and to design the rehabilitation program. Conclusion: The proposed model became routine and consolidated the physiotherapeutic action in the respective care unit. The physiotherapy team started working 24 hours a day. The model made possible the standardization of physical therapy assistance and greater safety for the severe polytrauma patient

Antecedentes. La rehabilitación es un proceso de reaprendizaje motor, que mejora el desempeño en términos de adquisición de nuevas habilidades y adaptación o refinamiento de habilidades aprendidas previamente. A pesar de este conocimiento, existen, considerablemente, pocos estudios que describan el aprendizaje motor después de un accidente cerebrovascular (ACV) y la relevancia del mismo en los procesos de rehabilitación y recuperación. Objetivo. Describir la transferencia del aprendizaje motor en pacientes con antecedentes de accidente cerebrovascular después de un tratamiento con procedimientos de reeducación funcional. Materiales y métodos. Se realizó un estudio descriptivo de dos casos con pacientes que recibieron tratamiento fisioterapéutico ambulatorio con los principios de reeducación funcional desarrollados en el Hospital de las Clínicas de la Universidad de São Paulo (Brasil), entre los meses de agosto y octubre de 2013. La transferencia del aprendizaje motor se determinó según los resultados obtenidos en la evaluación de balance funcional (Mini-BESTest). Se evaluó el test antes y después del tratamiento y se compararon los resultados para determinar el porcentaje de mejora. Resultados. En los dos casos se observó mejora clínica en el desempeño de la evaluación de balance funcional Mini-BESTest con un porcentaje de mejora entre el 21% y el 41%. Conclusión. Existen pocos hallazgos literarios que describen el mantenimiento de la capacidad de aprendizaje motor en pacientes con antecedentes de ACV. Este estudio reportó la capacidad para transferir el aprendizaje motor a una habilidad motora no aprendida (balance) en dos casos, con base en la mejoría clínica del desempeño motor en el test aplicado.


Background. Rehabilitation for patients is fundamentally a process of relearning, which improves the acquisition of new skills and the adaptation or refinement of previously learned abilities. Despite this knowledge, few studies describe motor learning after stroke and its relevance in recovery and rehabilitation processes. Objective. To describe the transfer of motor learning in patients with stroke history after a treatment with functional reeducation procedures. Materials and methods. A descriptive study of series of cases was conducted, with two patients that received outpatient physiotherapy treatment with the principles of functional reeducation at the Hospital de las Clinicas in the São Paulo University (Brazil) between August and October of 2013. The transfer of motor learning was determined according to results in the functional balance test (Mini-BESTest). The test was applied before and after the treatment, and the results were compared to establish improvement percentage. Results. In the two cases, clinical improvement was observed during the performance of the Mini-BESTest with an improvement percentage between 21% and 41%. Conclusion. Few research studies report the preservation of motor learning in patients with history of stroke. This study reported that two patients were able to transfer motor ability trained on the treatment to a similar untrained task (balance) based on the clinical improvements of motor performance in the applied test.

Objective: Evaluate the functional status of critically ill patients after hospital discharge and assess the effect of age and Acute Physiology and Chronic Health Evaluation II on the questionnaire scores. Study Design: An observational longitudinal study. Place and Duration of Study: The study was carried out between May 2010 and January 2011, at the Clinical Hospital of the University of São Paulo, Brazil. Methodology: 147 patients, admitted to the Intensive Care Unit, ≥18 years old, underwent at least 24 hours of Invasive Mechanical Ventilation (IMV), were eligible after hospital discharge for this study. The functional status was assessed by using the Barthel Index, Katz Index and Health Assessment Questionnaire – Disability Index (HAQ-DI), at one, three, and six months after hospital discharge. Results: Out of 147 eligible patients, 75 and 68 responded to the initial and final evaluations, respectively. Functional status improved significantly over time in all questionnaires. The analysis with two factors (age and severity of illness) in each of the three evaluations revealed significant interaction for Katz Index in the third evaluation (p=.001), and no interaction for the Barthel Index and HAQ-DI in each of the three evaluations. Conclusion: The functional status of critically ill patients after hospital discharge was not related to age or severity of illness or length of stay in IMV, ICU or hospital. When divided into groups of age and severity of illness, patients with less severe illness presented a higher Katz score if they were older. Yet older patients tended to present higher Katz score with more severe illness.

Introduction Evaluation of sit-to-stand and stand-to-sit activities is used by physical therapists in patients with neurological and musculoskeletal disorders. Sit-to-stand activity presents different descriptions of phases and movements; however the phases of stand-to-sit activity have not been established yet.Objectives To describe the movements during stand-to-sit activity and create an evaluation protocol.Materials and methods Stand-to-sit activity was described on anterior and lateral views based on the observation of 27 healthy subjects. The body segments chosen to analyze were feet, ankles, knees, hips, pelvis, trunk, spine, upper limbs, head and cervical spine. The movements of body segments were described as adduction and abduction, eversion and inversion, valgus and varus, neutral position and asymmetry. The protocol was assessed with questionnaires answered by 12 physiotherapists experts in the area.Results Stand-to-sit activity was divided in 4 phases: 1- "Neutral position", 2- "Pre-squat", 3- "Squat" and 4- "Stabilization". Two models of protocols were developed considering 5 body segments to the anterior view and 7 segments for the lateral view.Conclusion Stand-to-sit activity was described in 4 phases with sequential movements of each body segment. These protocols allow physiotherapists to identify unusual movements of body segments during the stand-to-sit activity.


Introdução Levantar e sentar são movimentos comuns da vida diária, e sua avaliação é utilizada frequentemente por fisioterapeutas em pacientes com disfunções neurológicas e musculoesqueléticas. A atividade de levantar apresenta diferentes descrições quanto às fases e movimentos, enquanto a atividade de sentar ainda não apresenta suas fases estabelecidas.Objetivos Descrever os movimentos durante a atividade de sentar e elaborar protocolos de auxílio à avaliação.Materiais e métodos Esta atividade foi descrita nas vistas anterior e lateral com base nos achados de um levantamento bibliográfico e na observação das filmagens de 27 indivíduos funcionalmente independentes. Os segmentos corporais analisados foram pés, tornozelos, joelhos, quadris, pelve, tronco, coluna lombar, membros superiores, coluna cervical e cabeça. Seus movimentos e condições foram descritos: adução e abdução, eversão e inversão, valgismo e varismo, posição neutra e assimetria. Os protocolos foram avaliados por meio de questionários de avaliação, respondidos por 12 fisioterapeutas especialistas na área.Resultados A atividade de sentar foi descrita de acordo com o estabelecimento de 4 fases. 1- “Posição inicial”, 2- “Pré-agachamento”, 3- “Agachamento” e 4- “Estabilização”. Foram elaborados 2 modelos de protocolos, sendo considerados 5 segmentos corporais para a vista anterior e 7 segmentos para a vista lateral.Conclusão A atividade de sentar foi descrita em 4 fases, e em cada uma ocorrem movimentos sequenciais de cada segmento corporal. Estes protocolos permitem identificar as alterações de segmentos corporais durante a atividade de sentar por meio da comparação dos movimentos descritos. A classificação do grau de inadequação corresponde ao número de segmentos corporais com movimentos alterados.

ABSTRACT

OBJETIVO: Verificar a associação entre a capacidade de executar as atividades instrumentais de vida diária (AIVDs) e a força de preensão palmar em idosos sem comprometimento cognitivo internados em enfermaria geriátrica. MÉTODOS: Estudo transversal. Foram incluídos pacientes idosos, com idade igual ou superior a 60 anos, de ambos os gêneros, admitidos em enfermaria de Geriatria. Foram excluídos indivíduos com doenças neurológicas, osteomusculares agudas e respiratórias exacerbadas; com indicação médica de repouso ou com uso de algum aparato clínico, bem como aqueles com déficits cognitivos e/ou delirium. Os dados foram coletados do prontuário e foi realizada a avaliação da capacidade de realizar atividades instrumentais de vida diária, por meio da Escala de Lawton, e a avaliação da força de preensão palmar, por meio da dinamometria do membro superior dominante. Os dados foram descritos em média e desvio- padrão. O teste de correlação de Pearson foi aplicado para verificar associação entre a capacidade de realizar AIVDs e a força de preensão palmar. RESULTADOS: Participaram do estudo 12 idosos (75% mulheres) com idade média de 80,67±6,35 anos. As médias da força de preensão palmar e pontuação total da Escala de Lawton foram 11,96±6,71 quilogramas-força (kgf) e 22,58±1,67 pontos, respectivamente. Verificou-se associação moderada e positiva entre a força de preensão palmar e a pontuação na Escala de Lawton (r=0,640; p=0,025). CONCLUSÃO: A capacidade de realizar AIVDs e a força de preensão palmar apresentaram associação na amostra estudada. .


OBJECTIVE: To verify the association between the ability to perform instrumental activities of daily living (IADL) and handgrip strength in hospitalized geriatric patients without cognitive impairment. METHODS: Cross-sectional study. Elderly patients aged over 60 years of both genders admitted to geriatric patients ward were included. On the other hand, elderly with neurological, musculoskeletal and respiratory acute exacerbated diseases; individuals with medical indication of rest or use of clinical apparatus, cognitive impairment and/or delirium were excluded. Data were collected from medical records. The ability to perform IADL was assessed by Lawton Scale and the handgrip strength was measured using handheld dynamometry on the dominant upper limb. A descriptive analysis with mean and standard deviation was carried out. The Pearson correlation test was applied to verify association between the total score of Lawton Scale and handgrip strength. RESULTS: The study included 12 patients (75% women) whose mean age were 80.67±6.35 years. The mean of handgrip strength and total Lawton Scale score were 11.96±6.71 Kgf and 22.58±1.67 points, respectively. A moderate positive association between handgrip strength and Lawton Scale score (r=0.640; p=0.025) was found. CONCLUSION: The handgrip strength and IADLs were associated in this study population. .
